Professional Referrals
We accept referrals from the NHS and private practitioners. We currently have referral pathways in place with a number of different services including East Lancs Parkinsons Nurses and Pulmonary Rehab which is supported by East Lancs CCG.
Exercise and Rehab Class Referrals
If you are referring clients into the Exercise/Rehab classes, they must meet strict criteria to ensure they are safe to exercise. Please contact us via email and we will be happy to share this with you.
Who would benefit from a referral for exercise and rehab classes?
Our services are ideal for anyone who has completed their clinical intervention, and are looking for a specialist, closely-monitored environment for exercise/rehab. In this environment they can continue and progress from the intervention. We are often described as the first step between NHS input and generic community exercise/activities.
Data Protection
In order to uphold Data Protection GDPR, we ask that you contact us by phone to refer a client/patient. We appreciate this can be time consuming, and are happy to take this referral from your admin support. However, we do require a named practitioner to verify the client is safe to exercise and who is point of contact if additional information is required.
Triage/Assessment
Prior to clients accessing our Exercise/Rehab, they are initially triaged over the phone, and given appropriate options of classes/treatments. We have a variety of classes which change periodically dependent on client need, so don’t worry if you are unsure of the individual classes we offer. If you think a client would benefit from continuing or starting exercise, we will guide them and advise on the class that is the right fit for them.
Onward Referrals
We work closely with other agencies and services such as Leisure services, local gyms, Age UK. When a client is referred to us who would benefit from another service, we signpost them onwards.
Complex Patients & Co-morbidities
As a specialist service working with health conditions, or sedentary lifestyle, we often come across clients who have more complex needs. Should you be working with a client and are unsure of their suitability, please get in touch. We are happy to discuss potential options over the phone and will endeavour to meet the client’s needs where possible or provide alternatives.
Anxious & Nervous Clients
Taking that first step can feel very frightening for many, especially with a health condition. We are aware of many of the anxieties experienced by clients before they start a class and how important this initial step is. We also know of the input from yourselves that has gone into working with the client already and we want to support the client to build on this. Therefore, we are happy to discuss clients, and go above and beyond to make the transition from yourselves to us as smooth and relaxed as possible.
Holistic Approach
At Appetite for Wellbeing, we are very aware the impact emotional health can have on presentation of physical health. Due to this, we regularly review clients progress during their sessions, and have team meetings to discuss their individual needs. We will always talk to clients about additional options that we might consider beneficial to them.
As well as offering services in-house, clients may be referred back to yourselves. Alternatively, an outward referral may be made to the relevant service in agreement with the client.
Referrals for Wellbeing Support
In addition to our Exercise/Rehab, we offer a range of other services. For example, we have counselling as well as other treatments that your client may benefit from emotionally. For many, it can feel daunting to pick up the phone to ask for help, so we take referrals from professionals where this may reduce the stress for the client. If you can refer your client over the phone to us, we’ll contact them directly. Of course, we can only take referrals that the client has consented to. We also ask that you make your client is aware who will be contacting them.
